This form is only for those that wish to add a digital hub to the national database of physical locations that patients can utilise to access Near Me appointments.
To request your digital hub details are added to the national database, please complete this application form and choose the relevant options. If you have any difficulties completing it, or have any questions, please call us on 01224 816666.
Our team will respond to let you know when the new digital hub has been added.
